PE Uniform Order Form

Complete One Form Per Student

PE is a required course for Middle School students and uniforms are mandatory. You may pre-order uniforms online at any time or stop by the School Store beginning August 21st (8am-1pm) to purchase one. If you pre-order before August 17th, the uniform will be delivered to the student’s advisor and distributed the first week of school. Students are not required to dress out the first couple days of school.

The following is a guideline for sizing and all shorts have a drawstring waistband:

Youth Medium 70 - 85 pounds
Youth Large 85 - 100 pounds
Adult Small 100 - 120 pounds
Adult Medium 120 - 140 pounds
